Case Study 1
The patient in Case Study 1, represents an interesting challenge. She had breast fed two children and
presented with two issues: (a) she is very thin - note the ribs and collarbones are showing and (b) she has m oderate sag. Ideally, for a thin patient, the implant is placed in a sub muscular pocket to produce a natural appearance in the breast upper pole. However, with m oderate sag, the implant is best placed above the m uscle to better fill out the sag in the lower pole. For this patient a dual pocket procedure was perform ed, allowing the upper part of the im plant to lie under cover of the m uscle, but allowing expansion of the lower pole free of the m uscle. The infram am m ary fold was lowered to centre the im plant on the breast m ound. A high profile im plant was chosen to further elevate the nipple position This com bination of techniques allowed the creation of an attractive breast without the need for additional scaring associated with a breast uplift.

Case Study 2
This patient presented with a full athletic figure and she wanted a large, natural looking breast to balance her figure. To achieve this a round gel implant was placed in a sub muscular pocket, centred behind the nipple. (A small liposculpture was perform ed on the outer thighs to harm onise her figure.)

Case Study 3
This patient is thin with very little breast tissue. However, she wished for a very natural sloping breast ? a difficult look to produce in such a patient. To achieve this, a teardrop shaped im plant of m oderate projection was used in a sub m uscular pocket, with the im plant centred to give greater fullness in the lower breast pole.

Beautiful breasts can be created in any size, large or small (this is the patient's choice). However, an understanding of balance and harm ony can help create both a beautifully proportioned figure as well as a beautiful breast. For m any patients, beautiful breasts are easy to create because the characteristics of their pre-existing breast tissue lend itself to good results. However, to create beautiful breasts in other patients requires m uch greater skill, experience, and judgem ent. For exam ple, very thin patients, patients with tubular breasts or breast sag. For these patients the breast augmentation needs to be even m ore carefully and thoughtfully planned. The aim is to create the very best possible result for each and every patient. W hen I see a patient for the first tim e I exam ine and m easure her breasts and torso so that I know what I am working with. I then spend tim e to carefully go through my patientÍs desires and the look she wants, for exam ple a very natural breast or the 'Californian beach girl look'. We go through photographs to allow the patient to show me her wishes in terms of breast size, shape, nipple positions, upper pole fullness, cleavage, projections, etc. Having understood her desires, I develop, in my mind's eye, an image of the breast I intend to create. The procedure is then designed, tailor-m ade to achieve this desired result. The plane is defined (pre or sub m uscular), the position of the im plant on the torso, the height of the infram am m ary fold (fold under the breast), and the shape, size and projection of the im plant. I also define additional techniques that need to be employed, such as release of tubular breast cone, lowering of the infram am m ary fold, nipple elevation procedures, etc. For some patients their pre-existing breast characteristics will not allow them to achieve the results they desire. These patients need careful guidance to understand the results they can achieve so that they have realistic expectations. The creation of beautiful breasts requires the doctor to be both artist and surgeon, and to carefully listen to his patient's desires and to tailor the surgery to create the desired result.
