1. Who is Breast Augmentation for?
Liposculpture has changed enormously over the years to become a procedure with minimal recovery and predictable results. Sydney cosmetic surgeon DR ANOOP RASTO G I explained to Jodi Thiessen
There has been a popular attitude that people should be happy with their appearance the way it is. "Fo r somebody who has breasts they are unhappy with they can see very clearly how that is an unreasonable attitude to have,"said Sydney cosmetic surgeon Dr Anoop Rastogi. "It is important to understand how self conscious some people feel and how much confidence they lose in their social interactions because of concerns they have about their appearance. "There are four major groups of women that seek augmentation and the surgeon needs to devise an operation and choose an implant for each individual.
1. Women who are younger or older who have always had small breasts and have aspired to have larger breasts." For this group of patients the surgery is relatively straight forward and the decisions are based on the size the patient would like to achieve," said Dr Rastogi. The size the patient desires is tempered with what is possible to produce given their anatomy. A surgeon with artistic skill can help the patient by giving them an indication of w hat w ill give proportion to their figure. "This group of patients need a lot of guidance ,help and education. So its very important for the surgeon to spend time with each patient discussing what is possible," said Dr Rastogi.
2. Women who have had babies who m ay or may not have been happy with their breasts before,but since having babies their breasts have sagged,become smaller and empty. Most of these women find that their breasts are empty,particularly in the upper part . This group is often a lot more aware of w hat size they want to be because they have seen them selves with larger breasts while breast feeding. This surgery has more to do with choosing the type of implant that will produce the shape they want,"explained Dr Rastogi.
3.Women who have had nice breasts during their youth but as time progresses or with weight gain or loss they have become empty and saggy .In the past this group of women may have been told that the only way to treat sagging breasts was through an uplift. However, often they can be treated with a very carefully designed breast augmentation alone. "Sometimes we can rotate the nipples upwards by the way we position the implant and by choosing a shape and size that w ill help .We usually choose a slightly higher projection implant that will take up more of the skin lower down the breast and give it the appearance of being lifted,"said Dr Rastogi. However, there still comes a point where a breast augmentation alone will not be the right procedure. If the patient is unsure,seeking a couple of opinions from different surgeons will be beneficial before undergoing any surgery .
4.Women who are unhappy with the shape of their breast. The two major problems women have with the shape of their breasts are unevenly sized breasts and tubular b re a s t s .Dr Rastogi explained:" Its very common for women to have different sized breasts. In fact its normal, just like you always have a larger foot. It when that difference becomes moresignificant that it becomes a concern . Usually we put an implant in to both breasts and either vary the size of the implant or do a small reduction or liposuction on one side, "said Dr Rastogi." Tubular breast deformity is difficult to treat, but can often gain an improvement with breast augmentation when specific techniques are used,"said Dr Rastogi. "W e score the underside of the breast so the cone opens up like an umbrella and we put the implant behind. To some degree this can correct the tubular shape." It is important for any patient,regardless of the reason they want breast augmentation,t o be able to communicate and openly express what they want to the surgeon . Once this has been achieved the patient must be confident the surgeon understands what they wantand has the technical skill to produce that for them .If what they want isnt achievable for them then the surgeon needs to discussthis with the patient so their expectations are realistic and t h e y re not disappointed with the result. "It is a very personal experience. Patients have a lot of fear sand anxieties when they go to see a surgeon," said Dr Rastogi. "Its very important the surgeon is understanding and the patient and surgeon can relate and communicate openly." The surgeon needs more than technical skill-anartistic eye and understanding of what the patient is trying to achieve are also vital.
